Most women picture perimenopause as hot flashes and skipped periods. In practice, those are often the last symptoms to show up — not the first.

Across community discussions and published research, the earliest and most common complaints are far less talked about: sudden anxiety with no clear trigger, brain fog that makes you lose your train of thought mid-sentence, new joint pain, itchy or crawling skin sensations, heart palpitations, and a resting heart rate that seems to have crept up for no reason.

Sleep is usually the first domino. Even without night sweats, many women in early perimenopause start waking at 3 or 4 a.m. and can't fall back asleep — a direct result of shifting progesterone, which has a calming, sedative effect on the nervous system.

Cycle changes are also more subtle than 'periods stop.' Cycles often get shorter before they get longer, sometimes arriving every 21–24 days instead of 28. Flow can become heavier or lighter, and PMS symptoms frequently intensify even while the cycle itself still looks 'normal' on paper.

The reason so many of these symptoms get missed or dismissed is that a standard hormone panel drawn on a random day often comes back 'normal' — hormone levels in perimenopause fluctuate wildly, sometimes day to day, which is exactly what makes the transition so disorienting.

If several of these are showing up together — disrupted sleep, new anxiety, brain fog, and cycle changes — it's worth having a real conversation with a physician who treats this as a distinct clinical picture, not a collection of unrelated complaints.
